To integrate, share data, and put the citizen, pasient and peers in the centre of the treatment is a way to help local communities to cope with the growing burden of diseases. Rural areas have limited access to specialists and long travel distances, while we need to manage chronic conditions in aging populations. We also need to support informal caregivers and family members. That requires communication and coordination tools for relatives and home care staff.

How can we do more with fewer resources while maintaining quality and access in distant areas? We see that digital tools—available today and evolving rapidly—are empowering care teams, families, and patients.

We also need mobile workflows for field staff, with Integration of scheduling, medication management, and documentation

In this session we highlight one or two examples of rural or community hospitals using Epic to extend reach and improve outcomes. We also look at AI and predictive analytics for earlier interventions, and how we can engage family, caregivers, and social networks digitally.

The vision is that every patient, regardless of geography, connected through a seamless care network. Rural organizations should start small, scale smart, and partner for success
