Hospitals at Home in the Nordics
Over the past many years, the Nordic healthcare systems have focused on ensuring better and more effective hospital treatment through centralisation and specialisation.
This task has been accomplished with great success, resulting in hospitals in the Nordic countries today delivering care of the highest quality. However, the current organisation is not a sustainable response to the demographic and resource challenges facing healthcare systems. In all the Nordic countries, there is currently increased attention on moving patients and treatments out of hospitals and into citizens’ own homes, without compromising quality or patient safety.
The session will take stock of the work to move treatment into citizens’ homes, examine how far each country has come, and discuss the overarching policy strategies that underpin these efforts. Are the systems and citizens ready for change? What tools are available to support the process and measure progress? The session will feature presentations from Denmark, Norway, and Iceland as well as an introduction to the Himss Continuity of Care Maturity Model (CCMM) and a discussion of the needs and experiences with tools to support the process.
