INCA Health is an AI-powered digital health solution designed to improve the management of patients with multimorbidity (PMM) through early detection, coordinated care, and patient empowerment. It integrates health, social, and behavioral data to provide a multidimensional assessment and a predefined workflow for patient enrollment. The platform enables shared care plans between professionals, patients, and caregivers, ensuring that each actor accesses the right information at the right time. A multidisciplinary care center oversees patient follow-up holistically, supported by a clinical decision support system that uses AI to reduce unwarranted clinical variability and enhance coordination. From the patient side, INCA offers an app tailored to different profiles that promotes engagement, co-responsibility, and adherence through gamification and education. The project is developed by a consortium led by the Fight Infections Foundation, IGTP, the UOC, the Norwegian Centre for E-Health Research, and Doole Health, whose patient remote monitoring and telemedicine platform anchors the solution’s multichannel communication and virtual care capabilities.
