The INCA Solution offers an initial multidimensional assessment through personalised questionnaires for the early identification of Patients with Multimorbidity (PMM), integrating data from multiple sources, such as social, health and physical activity information. The platform facilitates the creation and management of Shared Care Plans (SCP), allowing centralised real-time coordination between different professionals, with differentiated levels of access according to role. In addition, INCA includes an alert system for the early detection of decompensation and a Clinical Decision Support System based on artificial intelligence to optimise care. It also incorporates a specific app for patients, caregivers and relatives, designed to promote empowerment and adherence to treatment through gamification.
This innovative solution was designed within a Consortium formed by 5 partners: Fight Infections Foundation (FLI), Germans Trias i Pujol Research Institute (IGTP), the Open University of Catalonia (UOC), the Norwegian Centre for E-Health Research (NOR) and Doole Health (DOOLE).
