v/Rachael Smith, Population Health & Care Programme, NHS
Rachael has over 20 years’ experience in programme management for the NHS. She currently leads the population health management analytics function within South East London NHS Integrated Care System, maximising ways in which healthcare data is used and ensuring good data quality is in place. This involves working with primary care, mental health, acute and community provider data to leverage actionable and transformative ways of managing patient care. Rachael focuses on developing service strategies and priority areas using the data available in the population health and care system to underpin and guide transformation into actual action.

Ben is passionate about supporting healthcare systems make sense of their health data and how it is used to transform care delivery at individual, regional and national levels. He is working closely with regional health authorities and national governments, to provide solutions and services that meet advancing health and social care needs.
